In an effort to provide the best care to all of our patients, we request that patients new to KMP Aesthetics arrive 15 minutes prior to scheduled appointment times to complete new patient paperwork. This allows us to keep our appointments running on time and wait times to a minimum.
New Patient Forms
If you prefer to complete your new patient paperwork ahead of time, we have included a link below for your convenience. Please print and complete the enclosed paperwork in its entirety and bring it on the day of your appointment. As an added convenience, the paperwork may be printed and filled in before you arrive to the office.
Appointment Reminder Preferences
At KMP Aesthetics, we seek to make it easy for our Seattle patients to remember their appointments. As a courtesy we provide a reminder for your upcoming appointment 24 hours in advance via text message and email. If receiving your appointment reminder is more convenient via e-mail, please let us know and we will gladly update your preferences. To request e-mail reminders, please fill out a request and we will update your request promptly, or let one of our staff members know at your next visit.
Appointment Cancellation Policy
In order to best serve our patients, we request 24-hour notice if you wish to reschedule your appointment. Appointments that are not rescheduled prior to the 24-hour notice window will be charged a $50 cancellation fee as we will be unable to offer that time to another patient. All new patients to KMP Aesthetics will be required to provide a credit or debit card number in order to secure their first appointment. The same cancellation fee will apply for new patients.
Join our Patient Recognition Program
We offer a fantastic opportunity for our Seattle-based patients to save money on treatments with our Patient Recognition Program. Our newsletter program will offer you, our valued patient, discounts on your favorite treatments like BOTOX®, JUVÉDERM®, Radiesse®, laser hair removal, IPL (Photofacial), and a wide array of other services. Once enrolled, you will receive a short newsletter, via e-mail, once a month, which will entail the specials available to you at that time. The savings are fantastic and other than the once a month, we won’t bombard you with e-mails. Please ask to be enrolled in our program and you will receive monthly cost-savings opportunities at KMP Aesthetics.

Turn RIGHT on Madison Street
Follow Madison Street up the hill
The street following Terry Avenue is Boren Avenue
Turn RIGHT on Boren Avenue
The next street is Marion Street
Turn RIGHT on Marion Street and immediately pull into the Cabrini Center Parking Garage
Once in the parking garage, follow signs for Cabrini Medical Tower
Park on level P2 or P3, if possible, and take the elevator to the 19th floor

Take I-5 South
Exit James Street
The first light is Cherry Street. Continue forward to James Street
Turn LEFT on James Street
Follow James Street up the hill
Turn LEFT on Boren Avenue (Boren Avenue immediately follows Terry Avenue)
Follow Boren Avenue to Marion Street
There is a landmark sign post on the corner of Boren Avenue and Marion Street labeled “Cabrini Center” with an arrow pointing to the parking garage
Turn LEFT on Marion Street
Immediately pull into the Cabrini Center Parking Garage on the RIGHT
Once in the parking garage, follow signs for Cabrini Medical Tower
Park on level P2 or P3, if possible, and take the elevator to the 19th floor
